In medicine and nutritional medicine it is known that people store fat in different places. Women and men have fat deposits in different places. The fat deposits serve as our natural energy store. The process of storage cannot be prevented and is absolutely natural. However, you can't choose where these spots are formed. Depending on the disposition, such fat pads can also negatively affect the appearance of the body contour. The size and shape of the cushions can be bothersome and misshapen.

There are various diets that are complex and require great discipline and possibly even a change in life. Despite diet and lifestyle changes, it is possible that certain cushions remain. These fat deposits can accordingly be very stubborn and gnaw at the self and body image and the ego, the self-confidence. In some cases there is a familial predisposition to poor metabolism, which leads to fat distribution disorders, the so-called lipodystrophy. Only one or different parts of the body can be affected. In the following areas of the body, the aforementioned accumulations of fat often occur.

  • Double chin - fat deposit on the neck

  • Breeches - fat depot on the side of the thigh

  • Belly fat - "satchel" - pot belly

In these situations and many others, liposuction is the best solution. The simplest way would be to remove the problem fat through an intervention without surgery. However, the previous methods such as the devices such as cryolipolysis are still far from delivering controlled and reliable results. With cryolipolysis, the fatty tissue is frozen and then dissolved. Unfortunately, uncontrollable scars and uneven, unsightly contours can occur in a significant number of treatments. Liposuction with the suction cannula of a liposuction device, on the other hand, is easy to perform, is offered routinely in Bern and Zurich and is of very good quality and effectiveness. Liposuction is no substitute for a balanced diet and naturally induced weight loss. Rather, it is used in a targeted manner when there is a disproportionality of the fat deposits in certain areas, i.e. when one sees a local excess of fat deposits. With liposuction, you can correct the misallocation of fatty tissue.

How does liposuction work?

First, after a general medical clarification, a specific examination of the disruptive areas by Prof. Shafighi in Bern takes place. Here the areas are photo-documented. Prof. Shafighi determines whether liposuction is the right method to resolve the problem areas. It is also determined which liposuction technique should be used. Conventional liposuction with tumescent solution, with vibration technology, with laser or with ultrasound. Prof. Dr. med. Shafighi explains to you that liposuction is done through small skin stitches using cannulas. Depending on the possibility, these stitches of approx. 3 mm are located in places that are not easily visible so that they do not interfere later, although it must be said that these are really the smallest stitches of 3 mm, just for access to the instrument to have.

Tumescent solution (local anesthesia = local anesthetic and epinephrine, which is a vasoconstricting medication) can now be filled through the small holes. This liquid is distributed and diffuses evenly between the lipocytes in the adipose tissue. This means that the affected areas are anesthetized and therefore numb and the patient no longer feels anything in the specific areas.

A cannula is now precisely inserted under the skin into the fatty tissue through the punctures. The liposuction takes place via this cannula. By moving the cannula back and forth, the fat cells are gently detached from the loose bandage in which they are naturally located and sucked off through the hollow space of the cannula like through a straw. The fat cells in the subcutaneous tissue can be imagined as grape-like, packed in thin connective tissue layers. They are about 0.5 to 2 cm tall. The sheaths are loose partitions, these are separated from the grape-like fat cells (adipocytes) by the movement of the cannula. The fat cells are suctioned off with the help of negative pressure. The necessary negative pressure is generated by a vacuum pump, which is an integral part of the liposuction device. Connective tissue, blood vessels and nerves are spared, which means they are not destroyed during suction.

When the surgeon has finished liposuction, the cannula is gently pulled out again and the small holes through which the cannula has passed are closed with 1-2 stitches (very fine sutures). Then we like to apply a somewhat firmer pressure bandage so that as little swelling as possible can occur. In principle, liposuction is not difficult, but experience is still required so that a good, beautiful and, in this case, even result is achieved with liposuction. It is or should be the art of the surgeon to feel through the skin with great sensitivity and assess how many fat cells should be removed and what needs to remain in place. After all, it is not least the fatty tissue that lies beneath the skin and is responsible for the shape and fullness of the body areas.

Usually a large part of the fat layer can be removed. But only up to a certain limit, then an individual residue of fat cells must remain so that no dents and irregularities arise. If too much fatty tissue is removed, the overlying skin can no longer compensate for this volume deficit. Liposuction should be performed by a specialist with the necessary experience. You don't need to do any special preparation before liposuction. Prof. Shafighi will be happy to talk to you about the upcoming procedure before the liposuction. Your ideas and expectations will be addressed and the area to be treated will be inspected.

After liposuction - liposuction

After liposuction, when the local anesthesia is reduced by the tumescent solution, there is usually slight pain. Small bruises (hematomas) are also not uncommon. Here we try to counteract this with a pressure bandage. For larger areas that have been treated with liposuction, we recommend wearing compression clothing. These include compression pants or elastic bodices. This reduces the formation of bruises and seromas (= accumulations of wound secretions). At the same time, it supports an even shrinkage of the skin over the suctioned area.

What also helps against postoperative swelling are ice packs to cool and swell. Pain and bruises go away in a few days, the hematomas usually take several weeks. After that, the skin looks normal again, because it typically automatically adapts to the new shape due to its own elasticity. Smaller internal fibrosis processes also lead to a desired shrinkage of the skin. Excess skin such as a fat apron is not to be expected after liposuction, there are no excess skin folds afterwards. The mobility will normally not change after the operation.

The personal well-being after the fat suction is usually very good. Exercise should be avoided for a few weeks after liposuction. Only lighter work should be done. Resting is recommended. Do not massage the liposuctioned areas. What Can You Expect From Liposuction? Treated patients report that liposuction corrected their body image very well and gently. In principle, the result would be permanent, with one condition. A renewed weight gain can also lead to a renewed storage of fat cells in the treated area. The later weight gain or weight loss happens proportionally to the entire body.

Liposuction works particularly well with local fat deposits on the hips, stomach, flanks, ankles, upper body and under the chin. We usually get excellent results in these places. Other regions can be treated according to the surgeon's judgment. Clearly defined fat pads are a good prerequisite, and liposuction is a suitable method for this.

If you have decided that you would like to have liposuction performed, we recommend that you are ideal to normal weight at this point. If the skin is tight and elastic, it can best lie nicely over the area that has been sucked off afterwards and gives a nice result. Of course, liposuction is not a weight loss tool. It cannot replace diet and is not a substitute for physical activity. However, liposuction can remove localized fat deposits and fat deposits. In these places, even extensive regular training is sometimes of no use.

We all know the example: saddlebags that annoy a young woman because she suffers from the fat deposits on the hips and thighs. Despite regular training and a healthy diet, these shape-defining cushions do not disappear. In such a case, a professionally performed liposuction can help restore your shape, figure and thus your body image. During the consultation, it can be checked whether a combination of liposuction with a tightening operation could bring an even better result. This can be used when a large amount of fat cells are sucked off and the skin's own elasticity, in order to regress on its own, is exceeded.

During the tightening operation (e.g. thigh lift , upper arm lift , tummy tuck ), part of the excess skin is removed. Before the procedure, Prof. Shafighi will discuss with you whether this is recommended. The result of liposuction is permanent, the fat cells do not grow in a compensatory manner and also do not increase in other places just because fat has been removed in one place. The body shape after liposuction is basically consistent and the success is sustainable.

Wann ist das definitive Endergebnis sichtbar?


Die direkt nach der Liposuktion auftretenden Schwellungen und möglicherweise Blutergüsse vergehen nach einigen Wochen. Das Endergebnis ist nach ca. 3 – 6 Monaten sichtbar.




Ich treibe regelmässig Sport, jedoch verschwinden die Fettpolster an der Hüfte (Reithosen) nicht, ist das normal?


Besonders die Körperregion Hüfte (Reithosen) speichert die Fettpolster sehr hartnäckig ein. An diesen Stellen ist es sehr schwierig, gezielt abzunehmen. Leider helfen bei Fettpolstern an den Hüften oftmals weder ausgiebige Bewegung noch gesunde Ernährung. Die Reithose ist eine sehr geeignete Stelle für das Fettabsaugen.




Gibt es Körperregionen, an denen eine Fettabsaugung besonders gute Ergebnisse hervorbringt?


Besonders gute Erfolge erzielt man beim Fettabsaugen bei Frauen an den Oberschenkelpartien der Innen- und Aussenseite (Reiterhosen). Ausserdem sind die Hüften und der Bauch hervorragende Kandidaten. Bei den Männern sind die Bereiche ähnlich. Bauch, Hüfte und die Brust können besonders gut und erfolgreich operiert werden. Bei Männern tritt mit der Gynäkomastie ein Phänomen auf, welches mittels Fettabsaugung beseitigt werden kann.




Bleiben nach dem Liposuction sichtbare Narben?


In der Regel sind die Narben so klein, dass sie nach vollständiger Abheilung nicht auffallen. Darüber hinaus achten wir darauf, die Minischnitte möglichst „versteckt“ zu platzieren.




Wie verhält sich das Gewebe bei einer Fettabsaugung? Wird eine Straffung bei der Liposuction erzielt?


Um eine Gewebestraffung bei einer Fettabsaugung zu erzielen, müssen mehrere Voraussetzungen gegeben sein. Eine Gewebestraffung ist meistens nur an bestimmten Regionen des Körpers möglich. Die zu behandelnde Haut sollte elastische und gesund sein. Sind diese Voraussetzungen gegeben, ist es möglich, mittels der Vibrations-Technologie und der Oberflächenmodellierung eine Straffung des Gewebes zu erzielen.




Können sich nach dem Fettabsaugen neue Fettzellen bilden oder sich an anderer Stelle Fettdepots entwickeln?


Solange das Gewicht gehalten wird, sollte sich nichts am Resultat verändern. Nehmen Sie jedoch extrem zu, werden sich auch Ihre Körperproportionen verändern. Die abgesaugten Problemzonen werden dabei jedoch weniger Fett speichern, da nach der Liposuktion nur noch eine begrenzte Zahl an Fettzellen verfügbar ist.




Welche Methoden des Liposuction gibt es?


In der ästhetischen Chirurgie sind zwei Methoden vorrangig in Gebrauch: Die eine stellt die feuchte Fettabsaugung dar, die andere die trockene. Unter der feuchten Fettabsaugung versteht man die Behandlung mit Injektion von Tumeszenz-Lösung. Wir empfehlen grundlegend Eingriffe unter der feuchten Methode durchführen zu lassen.




Was sollte bevorzugt werden: Vollnarkose oder lokale Betäubung mit Sedierung?


Wir empfehlen eine Durchführung der Fettabsaugung innerhalb einer örtlichen Betäubung, denn dann ist eine vorteilhaftere Spannung der Muskulatur gegeben, wodurch die Fettpolster besser abgesaugt werden können.




Wie viel Fett kann in der Regel bei der Liposuktion entfernt werden?


Pro Sitzung ist Fettabsaugen von 3 Litern im ambulanten Umfeld möglich. Sitzungen, in denen mehr als 3 Liter Fett abgesaugt wird, müssen stationär durchgeführt werden. Hier liegt das Maximum bei 5 Litern pro Sitzung. Weitere Fettabsaugungen müssen in zusätzlichen Sitzungen passieren.




Können mehrere Stellen in einem Behandlungsschritt abgesaugt werden?


Grundsätzlich ist das möglich, allerdings muss dies im individuellen Fall geklärt werden, da es abhängig von der insgesamt abzusaugenden Fettmenge ist.




Kann die Fettabsaugung ein Bierbauchproblem lösen?


Hierbei muss man ganz klar sagen, dass ein Fettabsaugen am Bauch nicht weiterhilft. Das Problem des Bierbauches kann nur mittels einer ausgewogenen Ernährung mit gleichzeitiger sportlicher Aktivität angegangen werden.




Ist eine Fettabsaugung sehr risikoreich?


Wie bei jedem operativen Eingriff muss man auch bei der Liposuktion mit Schwellungen und Blutergüssen rechnen. Ebenfalls ist es wahrscheinlich, dass Sie nach einem Fettabsaugen ein Gefühl von Taubheit an den behandelten Stellen wahrnehmen. Dies sind jedoch auch schon die wahrscheinlichsten Nebenwirkungen, die nach einer Liposuktion auftreten können. Eher selten sind Risiken wie Thrombose, Embolien oder Blutungen. Ebenfalls seltener stellt man nach einer Fettabsaugung Asymmetrien fest, die einen weiteren Eingriff erfordern würden.
